The following information must be completed for each animal you plan to exhibit so that the committee can prepare floor plans and provide the information cards for your exhibit.  If you are unsure which animals you are going to bring, please list all animals you MAY bring; this will allow us to prepare the proper ID cards and will also allow us to review the diversity of animals at the show.  Please do not bring any animals that you have not listed on this form.  Attach additional sheets if needed.
